会員登録フォーム　Membership Registration Form

日本国際保健医療学会の会員システムを利用するためには、会員登録が必要です。
登録が完了すると会員システムのURLおよびログインするためのIDとパスワードが、下記で登録されたメールアドレスに送られます。
必要事項を入力し、「Next」ボタンをクリックしてください。
